An e-cigarette, vape pen or other electronic nicotine delivery system (ENDS) heats a liquid of nicotine, flavoring, propylene glycol and other additives into an aerosol that you inhale through a mouthpiece.
The most consistent predictors have been descriptive norms of parents’ and close friends’ smoking, indicating that addressing close network members might be the most effective in interventions [27]. In our study, parental smoking reported by adolescents increased all types of susceptibility when analysed independently. In the multivariate models, it did not have clear associations with S-SN and S-EC among girls.
Currently, e-cigarettes are not mentioned in the Illinois Smoke-free Illinois Act, which prohibits smoking in virtually all public places and workplaces, but some local ordinances do include e-cigarettes. E-cigarette aerosol is not harmless; it can contain harmful and potentially harmful substances, including nicotine and cancer-causing chemicals. While e-cigarettes typically have fewer chemicals than regular cigarettes, researchers found e-cigarettes present their own unique health risks and can increase the odds of chronic cough, phlegm, bronchitis, and asthma. Some cigarette smokers have replaced traditional smoking with electronic cigarettes (e-cigarettes) or vaping, thinking it is not as dangerous. Glyoxal and methylglyoxal are formed by thermal degradation or oxidation of PG and VG87. Glyoxal is considered mutagenic, while the related compound methylglyoxal has been identified as a metabolite during glycolysis and is thus naturally present in the body. Methylglyoxal is also present in foods and drinks such as honey and coffee.

Participants took one vape from an e-cigarette, with at least 12 mg/mL of nicotine, or inhaled a conventional cigarette, every 20 s for 10 min. Blood samples were collected 1, 2, 4, 6, 8, 10, 12 and 15 min after the first puff, and nicotine serum levels were measured by liquid chromatography-mass spectrometry (LC–MS). The results revealed higher serum levels of nicotine in the conventional CS group than in the e-cigarette group (25.9 ± 16.7 ng/mL vs. 11.5 ± 9.8 ng/mL). However, e-cigarettes containing 20 mg/mL of nicotine are more equivalent to normal cigarettes, based on the delivery of approximately 1 mg of nicotine every 5 min [40].
What may be the most important message of all is that e-cigarettes and vaping come with many health unknowns, Dr. O’Connor adds. "You see plumes of what looks like steam coming out of people’s mouths on the street when they are vaping, and I think they assume it’s mostly safe, mostly water. But these liquids used in vaping are filled with all kinds of stuff [like nicotine, marijuana, flavoring agents, chemicals], and we don’t always know what else is in there," he says. When potentially risky behaviors experience an uptick in popularity, health researchers are never far behind—gathering data.
Reynolds’ Velo nicotine pouches, while holding just a 1.2% market share, has had a 25.8% year over year sales growth, as well as up 96.7% for the four-week period. Nicotine pouches have a 5.1% market share with sales up 73.4% over the year. Dr Goel underscored the importance of dispelling misinformation and promoting evidence-based approaches to tobacco cessation.

Federal, state, and/or local governments need to set clear standards on environmentally responsible e-cigarette waste disposal and hold the industry accountable for adhering to them. E-cigarette manufacturers introduced a new tobacco product that soared in popularity with little thought on how to responsibly dispose of the resulting tons of e-cigarette waste. The group then performed a statistical analysis to determine group differences in vascular function before and after vaping.
E-cigarettes hit the market in the early 2000s and have since soared in popularity, especially among teens and young adults. Once believed to be a "safer" way to smoke, vaping is now considered a public health crisis by many health groups. Stay up to date on the adverse health effects of smoking and vaping tobacco while gaining important guidance on how to counsel patients on the risks and quitting.
Again assuming a worst-case exposure of 300 puffs day–1 spread over 8 h, the levels of glyoxal exposure to a consumer would still be more than 40 times lower than the occupational exposure limit of 0.10 mg m–377,94,95. The high standard deviation for glyoxal and methylglyoxal observed in BB18 e-cigarette emissions is probably related to an analytical sample matrix effect and/or batch variability83,96,97. Despite the high standard deviation, the percentage reduction of glyoxal and methylglyoxal in both e-cigarette emissions relative to cigarette smoke was ≥ 97.0% and ≥ 94.5%, respectively (Table 3).
Juul’s C.E.O., Kevin Burns, who is fifty-four, has a friendly dad-who-loves-his-vacation-house demeanor. Burns described Juul to me as a "cigarette-killing company." Before he accepted the job, he said, he convened an informal focus group in his kitchen with his son, who’s in high school, and a few of his son’s friends. He asked them why they had these things, when they got them, how prevalent they were. "We have frustrations about how the product is glorified on social media," he told me. I admitted to Winickoff that I was probably endangering my lungs as a weed smoker. He gamely pointed out that I could look into ingested forms of marijuana, and noted that my brain was in a much more stable place than it had been when I was twenty-one.
Electronic cigarettes, or e-cigarettes, include e-pens, e-pipes, e-hookah, and e-cigars are known collectively as ENDS— electronic nicotine delivery systems. According to the FDA, e-cigarettes are devices that allow users to inhale an aerosol containing nicotine or other substances. Other studies show that newer e-cigarettes deliver higher doses of nicotine more efficiently, which could be a reason why these findings were observed. The authors acknowledge that more frequent use of e-cigarettes may also reflect greater determination to quit smoking combustible cigarettes and that their study did not evaluate risks of youth e-cigarette use. Over the next few years, the optimism over e-cigarettes waned as their popularity skyrocketed. Juul’s sales increased over 600 percent each year to become the best selling device on the market while I inhaled an atmosphere’s worth of vanilla vapor into my lungs.
Jabba and colleagues, including co-senior author Sven Eric Jordt, Ph.D., analyzed an e-cigarette product sold under the brand name Spree Bar, which comes in at least nine flavors and is listed as containing 5 percent 6-methyl nicotine. Study results showed the actual amount of the chemical was about 88% less than labeled. The e-cigarettes also included an artificial sweetener that is up to 13,000 times sweeter than table sugar, and an artificial coolant that mimics menthol’s effects. Talk to your patients, including youth and young adults, about the dangers of tobacco use.

Products marketed for therapeutic purposes (for example, marketed as a product to help people quit smoking) are regulated by FDA’s Center for Drug Evaluation and Research (CDER).
Next, we examined the e-cigarette emissions of flavouring compounds, together with acetic acid and propionic acid, as listed by the U.S. These compounds were not analysed in cigarette smoke because the Ky1R6F cigarette used in the study is an unflavoured US-blended cigarette and validated analytical methods for these compounds were not available. From the studied carbonyls, only acetaldehyde, acrolein and formaldehyde are included in the nine WHO TobReg priority smoke toxicants55. Relative to cigarette smoke, their percentage of reduction in the e-cigarette emissions was ≥ 98.8%. Liquid refills are sold in quantities of 10 mL to more than 30 mL (about two to six teaspoons) in a variety of nicotine strengths. The products come in flavors with attractive scents and inviting packaging.

These devices have various names, including e-cigarettes, e-hookahs, vaporizer cigarettes, vapes, and vape pens. They come in a range of shapes.
A 2018 study assessed the lung function of 10 people who had never smoked cigarettes immediately after vaping fluids, either with or without nicotine. However, the researchers concluded that, overall, vaping is thought to be less harmful to the heart than smoking cigarettes. However, they found that people who smoked conventional cigarettes and e-cigarettes were more likely to have heart disease. In a recently published randomised trial of 886 subjects who were willing to quit smoking [100], the abstinence rate was found to be twice as high in the e-cigarette group than in the nicotine-replacement group (18.0% vs. 9.9%) after 1 year.

A recent study found that most people who intended to use e-cigarettes to kick the nicotine habit ended up continuing to use traditional and e-cigarettes.
However, more research is needed to fully understand emissions from e-cigarettes and their impact on human health. These aerosols can contain potentially harmful substances — including nicotine, formaldehyde and metals — some of which cause cancer and other harmful health effects. Some e-cigarette brands and other nicotine products state they contain synthetic nicotine rather than nicotine made from or derived from tobacco.
"Vaping products containing nicotine are subject to federal laws that prohibit sales to people under the age of 21," said study co-author Sairam V. Jabba, D.V.M., Ph.D., a senior research scientist at Duke University School of Medicine. "Even with the current relatively low use of e-cigarettes among adults – 3.7 percent – health care costs are already substantial, and likely to increase in the future if youth continue to use this product," said Max. "Vaping products containing nicotine are subject to federal laws that prohibit sales to people under the age of 21," said study co-author Sairam V. Jabba, D.V.M., Ph.D., a senior research scientist at Duke University School of Medicine. The 2022–2023 NDSHS had a series of questions regarding policy measures designed to address e‑cigarette use. Support increased for all e‑cigarette policy options among the Australian population (Figure 6).
However, a single e-cigarette can be harmful to the body’s blood vessels — even when the vapor is entirely nicotine-free — according to a new study by researchers in the Perelman School of Medicine at the University of Pennsylvania. Tobacco products contain nicotine, which is highly addictive and can harm brain development as teens grow. Rural youth are at higher risk for harm, because they tend to begin using tobacco products at a younger age and use tobacco products more frequently. Products like e-cigarettes, vapes, and e-hookahs typically contain nicotine, which is highly addictive and can harm brain development as teens grow.
That’s about twice as many people as those who used other methods to quit smoking. Of people who used other methods, 91% kicked nicotine products altogether. People usually think vaping isn’t as bad as cigarette smoking, but the mist you breathe in still has nicotine and other harmful chemicals in it. All participants were compensated $5 via digital card per response (7 assessments total, maximum $35).
Of the participants who were not using e-cigarettes, only 5.8% had quit smoking altogether by the end of the survey, while 9.9% had stopped smoking daily. At the end of the survey, 28% of smokers using e-cigarettes daily had ceased smoking tobacco altogether, while 45.5% had ceased smoking tobacco daily. The AHA notes that while vaping liquids contain fewer contaminants than cigarettes, they are not entirely safe. A Foundation-led study presented at the 2024 Society for Research on Nicotine and Tobacco annual meeting found that monthly e-cigarette sales in California declined by 41% after statewide restrictions on the sale of flavored tobacco products took effect. However, the report also noted that the tobacco industry continues to market products to offset these declines, including non-menthol cigarettes that contain other cooling chemicals. Continuing to monitor flavored tobacco product sales is key to assessing policies like these and informing further policy development and implementation. Toxicity is not the only factor in considering risk to an individual or a population from exposure to ENDS emissions.
It can also reduce fertility, increase the chance of developing type 2 diabetes, and contribute to bone loss. Even people who "smoke but don’t inhale" face an increased risk of mouth cancer. By Robyn Correll, MPHCorrell holds a master of public health degree and has over a decade of experience working in the prevention of infectious diseases. Researchers spent decades studying the health consequences of secondhand smoke. It might be a while before we have a clear picture of how secondhand vaping can impact a person’s long-term health, especially for young children.
Enrollees in the program receive interactive daily text messages tailored to their sign-up date or their target quit date if they set one. Program users who set a quit date (which they can change) receive messages for at least one week prior to the quit date and for at least two months following the quit date. Those brands have been sold in the U.S. for years, awaiting FDA action on their scientific applications. To stay on the market, companies must show that their e-cigarettes provide an overall health benefit for smokers, without significantly appealing to kids.
E-cigarettes consist of a mouthpiece, an e-liquid chamber, an atomiser and a battery. The atomiser has a wicking material that draws the e-liquid onto a battery-powered heating coil. Optimal vapour production depends on an efficient supply of e-liquid to the heating coil, which is limited by the wicking and rate of e-liquid evaporation10,11,12. Power levels that produce aerosol beyond the ability of the wick to resupply the liquid to the coil may result in overheating of the atomizer coil and consequently overheating of the e-liquid10,11. Different types of wicking material, varying in size and shape, have been used in e-cigarettes3,13.
At least one of the three chemicals was detected in 47 of the 51 flavors tested. Diacetyl was detected above the laboratory limit of detection in 39 of the flavors tested. Acetoin and 2,3-pentanedione were detected in 46 and 23 and of the flavors, respectively. The association between student- and school-level factors and susceptibility to snus use. "From a public health perspective, we have always been concerned about dual-use of both traditional and e-cig products," said study lead author Marisa Bittoni, an oncology researcher at Ohio State University in Columbus. The U.S. Food and Drug Administration and the European Union are grappling with these issues as they decide how to regulate the products. Unfettered access could leave people vulnerable to unknown health hazards, but there is also the chance that greater restrictions might hurt folks who are trying to forgo conventional—and more dangerous—tobacco products.
Probably the worst thing a parent could do for their child would be to buy an e-cigarette under the misconception that this might prevent them from smoking regular cigarettes, Krishnan-Sarin says. She encourages parents to talk openly and freely about vaping—with the caveat that they provide accurate information. "I think the problem is that parents lose credibility if they say something to try and convince their child, who then finds out that it isn’t true," she says. Vape pen and e-cigarette explosions from overheated, defective and/or modified device batteries have occurred, causing injury and serious health problems to users. You may have heard the benefits of vaping are that it’s safe or that it helps you quit smoking. Some people vape because they think it’ll help them quit smoking if they’ve already tried other methods and haven’t been able to quit. Unfortunately, you may end up addicted to vaping instead, so the benefits of vaping for smoking cessation may not be as good as they seem.
A 2016 report indicated that many vape fluids contain flavoring agents at concentrations that may pose risks to users. It carries the same health risks as vaping with any other e-cigarette brand. There’s limited evidence to date of the long-term effects of vaping because we know the lung effects of vaping will take decades to develop.
For a summary of Iowa youth vaping/e-cigarette use statistics from the 2018 IYS, check out our infographic and factsheet. Adolescents are more likely to use e-cigarettes than any other tobacco product, and rates of e-cigarette use have risen sharply in recent years. Population-level interventions to reduce tobacco use include price increases, mass media campaigns, and smoke-free policies that include e-cigarettes. E-cigarettes, also known as electronic smoking devices (ESD) and vaporizer cigarettes, are battery-operated devices that emit doses of nicotine and non-nicotine vaporized solutions that are inhaled.
The program offers specially-trained coaches, real-time text or chat support, easy enrollment and youth-focused promotional and educational materials. The majority of cases reported to poison centers are due to accidental exposures in children less than 6 years old. This reflects the increasing availability of these products in the home due to use by guardians or older siblings. Additionally, in the last 5 years, the FDA has warned companies selling e-cigarette devices made in the shape of toys, foods or cartoons that specifically appeal to use by young people. Of this sample, 3,242 participants developed heart failure within a median follow-up time of 45 months. As e-cigarettes deliver flavorings, additives, and, typically, nicotine through an inhaled aerosol, they are harmful to both the user and those who are exposed secondhand.
Strong decisive action is needed to prevent the uptake of e-cigarettes based on the growing body of evidence of its use by children and adolescents and health harms. Public Health Consequences of E-Cigarettes reviews and critically assesses the state of the emerging evidence about e-cigarettes and health. This report makes recommendations for the improvement of this research and highlights gaps that are a priority for future research. Vape explosions have been linked to faulty batteries in vaping devices.
To truly help tobacco users quit and to strengthen global tobacco control, governments need to scale up policies and interventions that we know work. Tried and tested interventions, such as brief advice from health professionals, national toll-free quit lines and mobile and digital cessation services are recommended. Where economically feasible, governments should also consider promoting nicotine replacement therapies and non-nicotine pharmacotherapies for cessation. E-cigarette emissions typically contain nicotine and other toxic substances that are harmful to both users and non-users who are exposed to the aerosols second-hand.
According to the 2021 National Youth Tobacco Survey, more than 2 million U.S. middle and high school students reported using e-cigarettes in 2021, with more than 8 in 10 of those youth using flavored e-cigarettes. The mid-to-long-term consequences of e-cigarettes are not yet known, as it's a new product and has been sold for less than a decade in the U.S. While much remains to be determined about these lasting health consequences of these products, we are very troubled by what we see so far. The inhalation of harmful chemicals can cause irreversible lung damage and lung diseases.
Vape aerosols may also increase the risk of heart disease, lung cancer, and asthma complications. The substitution was especially evident among cigarette brands popular with young people aged 20 and under, suggesting that flavor restrictions may increase smoking among youth as well as adults. The immense popularity of electronic cigarettes, or e-cigarettes, among young people has led many policymakers to restrict the sale of flavored varieties. Proponents of e-cigarettes claim they’re safer than smoking because they don’t contain the more than 60 cancer-causing chemicals in tobacco smoke and are not combustible. But e-cigs still deliver harmful chemicals, including nicotine, the extremely addictive substance in cigarettes.
If you experience side effects from vaping, get advice from a stop smoking adviser or specialist vape retailer before you decide to stop using a vape as your quit smoking tool. Almost two-thirds of people who use a vape along with support from a local Stop Smoking Service successfully quit smoking. Vaping is not completely harmless and we only recommend it for adult smokers, to support quitting smoking and staying quit. It made sense – Altria, the bad actor formerly known as Philip Morris, maker of Marlboro cigarettes, owned a 35% stake in JUUL.
Electronic cigarettes produce an aerosol by heating a liquid that usually contains nicotine, flavorings and other chemicals. Richmond-based Altria previously took a $13 billion stake in Juul in 2018, when the brand controlled most of the U.S. vaping market. But Juul’s value plummeted after it was hit with lawsuits and investigations over its role in sparking a national spike in underage vaping.
As one addiction expert has said, "The modern cigarette does to nicotine what crack does to cocaine." Pax Labs discovered that by adding benzoic acid to nicotine salts, which occur naturally in tobacco, they could mimic a cigarette’s rapid nicotine delivery. Both British American Tobacco Plc and Altria are prodding the FDA for heightened enforcement on "illicit" synthetic nicotine electronic cigarettes in the U.S. vaping market. The companies have estimated those synthetic products represent about half of the overall domestic e-cigarette market.
I want to carry the stink and taste that won’t let me forget I’m damaging myself when I’m smoking. The way to quit isn’t through a device that made a nicotine hit easier, or fun. In other words, just because something is safe to eat doesn’t mean it’s safe to be inhaled. (Duh.) Vaping also seems to trigger potentially harmful immune responses in the lungs.
